“One Day at a Time: Living with Cystic Fibrosis”
Art Exhibit
CFRI and Breathing Room are sponsoring a community-based art exhibit at CFRI’s 2004 conference.
We look forward to exhibiting artwork from across the country, and possibly internationally. To be considered
for exhibition, each artist must fill out the following for each entry. (Entries are limited to 8 pieces per artist)

- Why did you choose this medium?
- How long have you been using this medium, or is this a "first" for you?
- How does this piece relate to your experience of Cystic Fibrosis?
- How will it impact you personally to have your art exhibited in this gallery?
- Please write a brief ( circa 5 sentences) paragraph that shares something more about yourself. For instance: something about your family, more information about how you relate to your art, something about your experience with cystic fibrosis...

2-dimensional art not to exceed 30”x40”, and sculpture not more than 30 lbs.
Each piece will be subject to our selection process. This process is two-pronged. Art will be considered on relevance to the theme “One Day at a Time: Living with Cystic Fibrosis” and also on artistic merit.
Please send in photographs of your pieces for consideration (no original artwork
will be accepted at this time, regardless of size or medium). Pictures may be 3 1/2 x5, 4 x 6, or 8 x10.
Digital images must be a minimum of 180 DPI. (Also, we will accept color photocopies of small < 8.5x11” original work)
Please attach a sheet with the above information (name, title, etc.) to EACH picture submitted.
Also include a self-addressed stamped envelope if you would like the pictures returned to you.

All entries must be postmarked by May 1, 2004. Selected artists will be notified by June 1, 2004.
If selected, pieces of art will be received at the CFRI office July 1 – July 15, 2004, and held until the exhibit. Each piece must arrive at the CFRI office
exhibit-ready. (terms of “exhibit ready” will be available after June 1, 2004).
It is the responsibility of the artist to arrange and pay for transportation of the art to and from the CFRI office for this exhibit.
CFRI, Breathing Room and their volunteers accept no responsibility for damage to the art prior to, during, or after the exhibit.
All artists are invited to attend the conference, provided they meet the required cross infection guidelines set by CFRI regarding the educational conference.
